Healthcare and medical aid when retiring to South Africa

Arrange healthcare around your actual cover history and treatment needs before cancelling an existing policy or setting a moving date. A scheme's headline contribution does not tell you when your benefits begin, whether a waiting period applies or how your regular care will be paid for.

Before you start

For an older adult, the useful comparison is a written, personal set of terms: contribution, any late-joiner penalty, applicable waiting periods, benefits and restrictions, and the start date. Couples should obtain terms for each person rather than assuming the same history or conditions will produce the same result.

Separate three questions

First, what evidence does the immigration category and receiving authority currently require? Second, what cover will the scheme or insurer actually provide? Third, how will your treating professionals arrange continuity of care? A document accepted for one purpose does not automatically answer the others.

Directive 10 of 2026 waives the specified immigration medical-report requirements for future and pending visa and permanent-residence applications under regulations 9(1)(c) and 23(1)(f). That is a waiver of the identified reports, not a waiver of medical-scheme underwriting, a guarantee of healthcare benefits, or a statement that every category's cover requirements have disappeared. [B05B06]

For the wider healthcare system, read the South Africa healthcare guide. This page focuses on cover continuity and the questions that become important when retiring with an established medical history.

Waiting periods depend on the person's cover history

Section 29A of the Medical Schemes Act permits different waiting-period arrangements in specified circumstances. It does not mean every joining member automatically receives the maximum waiting periods. The relevant facts include whether there was recent medical-scheme membership, how long continuous membership lasted, the gap before application, and any unexpired waiting period. [B05B07]

For a person without beneficiary membership in the preceding 90 days, the section permits a general waiting period of up to three months and a condition-specific waiting period of up to twelve months. Different rules apply where recent continuous membership ended less than 90 days before the application. Those branches also have distinct protections relating to prescribed minimum benefits. Do not apply one branch's treatment to everyone, or assume all prescribed minimum benefits are payable throughout every possible waiting period. [B05B07]

Ask the scheme to identify the legal basis, start and end dates, and benefit consequences of each proposed waiting period. If prior cover is relevant, provide accurate membership dates and supporting certificates early. A verbal assurance that “you are covered” is too vague to resolve a particular medicine, appointment or admission.

Previous overseas cover may need separate assessment

The regulations define creditable coverage for late-joiner purposes. Do not assume that every foreign insurance policy, national healthcare entitlement or private overseas plan counts as qualifying South African medical-scheme coverage. Equally, do not discard your records before the scheme has considered the precise history. Ask which periods it accepts, which it excludes and why. [B05B08]

Retain dated proof of membership, termination and beneficiaries. If a document shows only a household policyholder, ask the former provider for evidence identifying the person who was actually covered. Keep copies of what you submit and the written assessment. Waiting-period assessment and late-joiner calculations are related to cover history but are separate questions.

Ask for the late-joiner calculation, not a guessed premium

The regulations allow late-joiner penalties in defined circumstances for applicants aged 35 or older, subject to the definition, creditable coverage and exceptions. Age alone does not establish a particular penalty. Prior continuous membership and the timing of breaks may matter, including the specific pre-April-2001 provision. [B05B08]

Request a written calculation showing the age used, accepted years of creditable coverage, excluded periods, applicable band and the contribution portion to which the penalty is applied. The regulations address the portion related to the member or an adult dependant who qualifies for a late-joiner penalty; do not assume the same percentage is simply added to every dependent's contribution. They also provide for recalculation where relevant evidence is later produced. [B05B08]

This page does not calculate premiums or recommend a product. A quotation should distinguish the base contribution from a penalty so the cost and the reason for it can be checked.

Compare the terms that affect real care

Comparison of Ask for in writing, Why it matters
Ask for in writingWhy it matters
Membership start date and any waiting periods for each personMoving day and benefit start day may differ
Treatment of existing conditions and prescribed minimum benefitsA general statement about cover may omit the applicable waiting-period branch
Regular medicine benefits, formulary and approval processA familiar medicine is not automatically funded on identical terms
Provider networks and designated service providersPreferred clinicians or hospitals may have different payment consequences
Co-payments, limits and out-of-pocket exposureContribution alone is not the household's possible healthcare cost
Late-joiner calculation and accepted prior-cover evidenceThe assessment must reflect the individual's records

CMS explains that prescribed minimum benefits have a defined scope and may involve designated providers, protocols and formularies. They should not be presented as unlimited payment for every service or every emergency bill in every membership circumstance. [B05B09]

Answer medical and membership questions accurately. If a question is unclear, obtain clarification rather than omitting a condition because it seems unimportant. Send sensitive health records through the recipient's appropriate secure process, not a general immigration enquiry form.

Build a continuity plan with your clinicians

List current medicines, routine appointments, monitoring, specialist referrals and any planned procedure. Ask the treating professionals what records and arrangements are needed for a safe handover. Ask the proposed local provider whether it can accept the patient and how billing works with the proposed cover.

Do not stop or change treatment to fit an application date or insurance assumption. Any clinical decision belongs with a qualified treating professional. If there may be a gap, establish how care and costs will be handled before cancelling the previous arrangement. A bridging product's name does not prove it covers existing conditions or replaces a medical scheme; read its actual terms separately.

A worked preparation example

Illustration only — fictional people, not an approval or treatment story. Peter and Leila intend to retire together. Peter has continuous prior scheme records; Leila has overseas cover and a regular prescription. They initially compare only a combined monthly quotation.

They instead request separate waiting-period and late-joiner assessments, supply dated cover records, and ask how Leila's medicine would be funded from the proposed start date. Their clinicians identify the handover records needed. The outcome remains open: the example shows questions to resolve, not that either person receives immediate benefits or a particular premium.

Common questions

Will a pre-existing condition automatically prevent membership?

Do not substitute that assumption for the scheme's lawful membership and underwriting assessment. Ask for the specific terms and reasons, including any permitted waiting period. The existence of a condition does not tell you the full result for every person or scheme. [B05B07]

Are waiting periods and late-joiner penalties the same thing?

No. A waiting period concerns access to benefits for a period; a late-joiner penalty concerns contributions under the applicable rules. Ask for both assessments separately. [B05B07, B05B08]

Does the immigration medical-report waiver remove a scheme's questions?

No. The directive concerns specified immigration reports. A scheme's lawful information and membership assessment is a different process. [B05B06, B05B07]

What if the assessment seems wrong?

Request the reasons and check the dates and documents used. Follow the scheme's complaints process and the CMS complaints guidance if unresolved. CMS's consumer guidance explains that route; an immigration enquiry does not replace it. [B05B10]

Prepare the next action

Gather each person's cover chronology and request written terms before setting cancellation dates. Keep a separate private clinical handover plan. Then check how the immigration application's evidence requirements fit the proposed arrangements.

Continue with retirement planning, the temporary retired-person route and moving to South Africa.
